RESUMO

The authors used Midazolam in dental treatment of 225 patients of both sexes (ASA I-IV). 25 patients suffering from heart diseases (ASA III-IV) were ECG monitored during the treatment. Midazolam with the biological half-life of 0.5-1.5 hours was successfully used for premedication of dental out-patients. The used doses had no significant influence on hemodynamic and respiratory values. Specific antagonist Flumazenil reduced the period of psychological recovery of the patients and thus made it possible for the patients to be dismissed within 30 minutes after the dental treatment if accompanied.

RESUMO

A description is given of the bioactive glass-ceramic material, BAS-O, and of clinical tests of this material in stomatology. Granules of BAS-O bioactive glass-ceramic were used for implantation into cyst cavities in jaw bones. Radiological examination was carried out after 3, 6 and 12 months. A total of 37 radiographs of 23 patients were recorded and evaluated. Symptoms of ossification were found in 28 cases (75.7%), while five radiographs (13.5%) did not show visible bone apposition and loss of the BAS-O granules from the operation site was found in four cases (10.8%). The authors have demonstrated the excellent healing properties and tolerance for the BAS-O material, and no significant changes were observed in biochemical and haematological examinations.

RESUMO

The bioactive glass-ceramic material BAS-0 and results of clinical testing of this material in stomatology are described. The granules of bioactive glass-ceramic BAS-0 were chosen for implantation into cyst cavities in jaw-bones. Radiological evaluations were carried out after 3, 6 and 12 months. Altogether 37 radiographs of 23 patients were evaluated. The symptoms of ossification were proved in 28 cases (75.7%), 5 radiographs (13.5%) were without visible apposition of the bone and in 4 cases the loss of the BAS-0 granules from operation wound was observed (10.8%). The authors proved very good healing properties and tolerance of BAS-0 material, no significant changes in biochemical and haematology examinations were observed.

RESUMO

At the group of heart surgery patients treated at the department of dentoalveolar surgery, II. stomatological clinic, Prague, the author found out that more than 3/4 patients had required dental treatment. Focal infection of odontogenous origin was impossible to exclude nearly in 80% patients. For one patient requiring dental treatment it came to treat 4.5 teeth of males an 4.1 teeth of females. Mentioned numbers are warning finding especially for physicians that indicate patients to heart surgery and for surgeons that realize these surgeries.

RESUMO

The authors submit information on the properties of a new macroporous hydrogel, which was synthetized specially for use in bone surgery. In animal experiments the authors evaluate microscopically the biocompatibility and process of biodegradation of the implant in osseous tissue.

RESUMO

The authors describe a case of polymicrobial endocarditis in a 24-year-old patient with immunodeficiency and a damaged heart (mitral valve affection, septal defect). Among the mixed flora, comprising Streptococcus intermedius and two types of the genus Bacteroides, in the haemoculture also a microaerophil potentially pathogenic type Eikenella corrodens was cultivated. The authors describe the properties of Eikenella corrodens, its occurrence in man and its ability to produce an infectious process in primarily damaged organs (in particular the endocardium) in weakened subjects and drug addicts. The authors mention the diagnostic criteria for assessment of Eikenella corrodens and how to cultivate it from infectious material.

RESUMO

Knowledge of dental surgeons regarding dental treatment of cardiosurgical patients was assessed by means of anonymous questionnaires at dental departments in Prague and in four district in the CSR. The tasks of the respondents was to suggest treatment in patients after implantation of an artificial valve and in patients with an implanted pacemaker. The doctors were asked whether they were informed on the problem and where.

RESUMO

The authors describe the case of a 24-year-old woman with valve disease. After a bout of respiratory tract infection, she was diagnosed to have bacterial endocarditis associated with mitral valve disease. The causative agents isolated included Eikenella corrodens, Streptococcus intermedius, Bacteroides oralis and Bacteroides bivius. At the same time, the patient was found to have developed IgA immunodeficiency. A complication accompanying the cardiac disease was spontaneous pneumothorax. Since antibiotic therapy had failed, the mitral valve was replaced by a prosthetic one. After the procedure, the patient had her teeth examined. The examination revealed complete destruction of tooth 36, thick layers of calculus and chronic gingivitis. E. corrodens was isolated also after microbiological examination of the patient's oral cavity. The reason for reporting on what we regard as an interesting case is that bacterial endocarditis with E. corrodens implicated as the causative agent is relatively rare; to date, polymicrobial endocarditis due to E. corrodens and other microorganisms has been described in intravenous drug addicts only.

RESUMO

A group of 61 patients underwent conservative and surgical dental treatment in general anesthesia and their blood was sampled for bacteremia before and after treatment. The antibiotic antibacterial prophylaxis was administered in 21 patients one hr. before treatment (both in conservatively and surgically treated patients). Immediately after the treatment the prevalence of bacteremia amounted to 80% in a group of ATB-free patients. In patients with ATB prophylaxis the prevalence of bacteremia after the treatment amounted to 17%.

RESUMO

In the course of 10 years the authors treated 1900 patients with dentition difficilis of the third molars. They preferred extraction of the tooth. They administered antibiotics to health subjects only when the extraction of the tooth was difficult and in perimandibular inflammations and in patients where general condition called for this treatment.
